The Brain Health Check cognitive assessment is conducted confidentially over the telephone. At completion of the 20-minute telephone call, a detailed report of your assessment will be sent to you by mail.

We do not ask for the name of your doctor or health insurance company and will not share your assessment results with any other party. Your test results will be sent to you alone and whether or not you share the results with your physician will be up to you.
